For improved coordination of your health care, it’s sometimes necessary to share your health information between your primary care doctor and supporting Care Team, specialist doctors, hospitals, outpatient centers, and clinical labs. Your chosen healthcare providers, suppliers, and vendors must be able to “electronically exchange” a summary of your health care information if you are referred to another provider or setting of care, both within and outside the ACO-ES. The Morehouse Choice ACO-ES takes the utmost care to ensure there are no unauthorized releases of your protected health information and that only those providers who have a specific reason or need to see your information will have access.
Equally as important is fostering a close relationship with you as a Medicare beneficiary to give you access to your own electronic medical record information so you can engage with your doctor and your supporting Care Team and share in decisions about your health. We make health information available to Medicare beneficiaries through secure electronic means, also giving you access to appointment reminders and other health information.
