We believe our ACO-ES can do better in the way we do business with you, as an important Medicare Beneficiary and as our patient. Namely, in the time you have to wait after you arrive for your appointment, the amount of time you spend with your doctor and other providers in the exam room talking about the way you feel, and getting the answers you need about your health and wellness. When you leave, we want you to feel like you want to come back to a Morehouse Choice ACO-ES health center if you need care again. We want to know what support you want to turn to if you are sick or just want to talk about how to stay well, stay healthy, or get healthier.
We believe you and your doctor should be leaders in charge of your health but just like around the house where you live, sometimes a little extra help can go a long, long way. So we put together what we call your Core Integrated Care Team and you can work with your Care Team to get extra health care service, (for example, lifestyle counseling or nutritional help) not only that you need, but that you want.
We foster shared decision-making. The Morehouse Choice ACO-ES affords Medicare patients the opportunity to be active partners in their healthcare. Our patient-centered approach empowers patients to prevent and effectively manage chronic conditions and avoid critical episodes wherever possible. The ACO-ES will prioritize communication and collaboration between patients, their families, caregivers, and/or authorized representatives and their Care Team, so that they are informed about treatment options, and their preferences and values are incorporated into the treatment plan through shared decision-making. This process is aimed at ensuring patient-centered care.

Speaking of quality standards, Medicare holds the ACO-ES to a set of quality standards for patient care and satisfaction that are monitored by Medicare on a regular basis. Over time, the ACO-ES’ quality performance track record will be made available to Medicare beneficiaries.
